i bought a stock last year and it changed it's name and a reverse split i use turbo tax I sold the stock this year what name do i use on the tax sheet when i bought it...?

I don't understand what exactly you are asking, if you are asking what name you should use for tax purposes this year, use the name it had when you sold it.
