Applications for anything, employment, welfare, whatever, should have:

• first initial - no way to tell sex of applicant
• middle initial - no way to tell sex of applicant
• last name - got to have some identifier even though ethnicity might be determined
• SSN
• no place for age or date of birth - just a place to affirm that you meet the minimum age requirement
• no place for gender - can't tell the sex of the applicant
• no place for ethnic background - no discrimination or advantages given
• address
• qualifications

No advantages or disadvantages for anyone. Level field for everything.
