Have you ever looked at the CIA report generated on 13 June 1967 (declassified and cleared for release in June 2006)? The linked report states that prior to the attack (about 3 hours prior) the Liberty had there orders modified to remain 100 miles away from the coast (instead of the initial 12.5 mile limit). Liberty claimed they never received such orders (report states the reason being "misunderstanding of responsibilities for delivery").
