You can google it or ask a local realtor, they have them, just use some white-out for their information.

I think I still have one from when I did the rentals for the boss. Give me a fax # and Ill fax it over to you.

Go to the real estate lawyer you would use in the event that things go badly. My lawyer, Janice Briscoe gave me one free after I asked her. I wanted to use an agreement that she was comfortable using/defending since she's the one I'll be using in court to defend me against a tenent if things go wrong.

I didn't read the lease thoroughly, but you can addendums to the lease, like a non-smoking clause. Although our realtor said it's hard to enforce that one. Our lease also states that the carpets have to be professionally cleaned when vacating (can't do it yourself), tenant has to show proof of receipt.
