My W2s Are Wrong??? I Think

Not sure if anyone can help me.
First.... I have pre tax AFLAC AND SIMPLE IRA.

But my numbers on my W2 box 1, 3, 5, and 16 are all the same.

This is wrong...I think?

Does box 12 have something? That should be the difference between box 1 & box 3. Box 14 shows fringe benefits.

Yes - it has my IRA
and box 14 in the OTHER...has my AFLAC.
