Barnacle said:When I changed my name for social security when I got married, I did it at the Office on Aging behind the Governmental Center in Leonardtown. Call them... I'm sure they know when they are scheduled to be there. It was VERRRRRRY crowded. I had to wait a couple of hours...and then the whole thing once I got up to the desk took about 3 minutes. Bring your marriage license (or divorce decree??) with you.

So, how does one petition the court? Just go in and talk to the clerk?
And which court would one go to? Civil, District?

It is a civil action in Circuit Court.Toxick said:I actually need to know this information as well...

The only problem with that is that you have to mail them original documents. Meaning original birth certificate and original marriage license.CityGrl said:You can also go online, print off the paperwork, and mail it in to the Social Security Administration.
Here's the info. and the form: http://www.ssa.gov/online/ss-5.html
