Nuclear sub crew faked inspection records

" WASHINGTON (CNN) -- Six Navy personnel on board the nuclear-powered submarine USS Hampton have been punished for forging inspection records for the cooling system of the ship's nuclear reactor, Navy officials said Monday.

The misconduct was discovered on September 17 but was made public after completion of an initial investigation.

One officer and five enlisted personnel received a "non-judicial punishment" after other Navy personnel discovered their actions, Navy officials said.

The crew neither maintained inspection records nor conducted the required inspection of the chemical levels associated with the cooling system, the Navy officials said. The crew then went back and falsified existing records to make it appear the work had been done, the officials added.

"There is not, and never was, any danger to the crew or the public," the Navy said in a statement.

Guess no one remembers the Thresher...
I remember that all of us were required to watch the USS Forestfire movie every year to remind us about how important fire prevention is at sea. I thought submariners had to watch the Thresher films.

The Navy Court of Inquiry concluded that the Thresher's loss was most probably due to material failure, not the result of crew errors during operations.
Yes, it was likely faulty welds which supports the original point of the importance of attention to detail. The lack of attention to detail can have horrible results wether it's during construction or if navy pers are gun decking inspections.

My dad was part of that investigation. It affected him deeply.... all those men... gone... horribly

So when I hear that some sailor was too lazy to do the job the way they are instructed... to me it's criminal... but that's just a brat's opinion...

My husband was in the Navy from 2000-2006, he was on a submarine for the first 4 yrs. I asked him about this, he said they watched the Forrestal footage in boot camp and the Thresher footage during Basic Enlisted Submarine School (8wks of training after boot camp and before going on to rating school, then his first command)

Before you blame the sailor...

Very often, guys on the deckplates are browbeat into signing off on checks by Division Officers or Department Heads who are more concerned with making their numbers look good than actually finding and correcting problems. I had a division officer once tell one of my junior guys to put current calibration stickers on my cooling skid gauges because we had INSURV starting the next day. Never mind that there was already a work request in with the gauge calibration lab to have the gauges done, or that it took, at a minimum, 30 hours to do the job... he just wanted it to LOOK like it had been done. Fortunately, the kid came and found me (nobody touched my radar equipment without me knowing for fear of being calibrated themselves) and I confronted the idiot division officer about it. Of course, he hung my guy out to dry and said that he never gave the order. He left it just vague enough to give himself deniability if/when the guy got caught.
