In context -

I like to let insurance companies know they're fired unless they produce satisfactory services - we need an environment in this country where insurance companies COMPETE.

And from personal experience in the government - I do like to fire people when they absolutely don't do any goddamned work, and you would not believe how hard that can be. If you don't do your job, it ought to be a law of the universe that you are automatically fired - but we work in an environment where a person's livelihood and comfort zone trumps what his employer requires - and that he has AGREED to do.
