PET Vanity tags or Humane Society tags

Does anyone know how to get these tags for your car? I've looked it up on the MVA website but that offers little guidance or help. I want to get new tags this month as mine are due to expire, I would like the PET (spay & neuter) or Humane Society on them and I know that a donation is mandatory which is not a problem !!
