Transcription errors
If you used to transcribe, then you probably know about Dragon, the voice recognition transcription system. It had just come onto the scene in the ER and didn't always have good recognition or have all the newest drugs available in the vocabulary. The glitches were still being worked out.
I went to discharge a guy once and was going over his dc instructions and prescriptions. The doc had written an rx for Cialis for erectile dysfunction. Dragon had transcribed this as: See Alice for erectile dysfunction. I got choked trying not to give it away in front of the patient and excused myself for a minute. Went and showed it to the doc. Cialis was a new med then and Dragon couldn't recognize the word. The doc went back into Dragon and spelled it out. Discharge back on track.
I used to transcribe for St. Mary's Hospital, so I was all too aware of who the local freaks were. One was a business owner whom I went to school with. I'll never be able to look him in the eye again.
