Universal Medication Form

St. Mary’s Hospital is pleased to announce an initiative for all patients to use one form for medications. This free Universal Medication Form can be folded and stored in a wallet for easy access. Patients can fill out their immunization records and list all medications, including prescription, over-the-counter and herbals. This simple and easy-to-use form is good for emergencies and for any time patients need to quickly recall immunizations and prescriptions.

The form is available through Health Connections and for download online on the SMH website. Log on to St. Mary's Hospital - Leonardtown, Maryland - Medical Services for the Southern Maryland Community of St. Mary's, Calvert and Charles Counties., click on the Patients tab at the top of the screen, then click on the Universal Medication Form on the left-hand side. Or, download the form directly at http://www.smhwecare.com/Documents/universalMedicationForm.pdf.

FYI- If your using Firefox it may crash when you click the links. It crashed on me 3 times before I finally opened IE. :doh:

I'm using Firefox 3 - I had no problems with either of the links.

PS - It's a pretty nice little form even if you don't use St. Mary's Hospital. I have a hard time remembering stuff for myself. Might come in handy for keeping track of other family members too if it's needed in an emergency.

:confused: I do too, and every stinking time I clicked it crashed on both links. Weird!
