Very Sick Dog

Hi guys,

I'm running out of time to save my dog and my vet and I are running out of ideas. I'm hoping someone not involved has some ideas into this 'mystery diagnosis' deal so I can run them by my vet tomorrow and maybe come up with something. He is a 7 year old 70lb neutered basset hound.

He was diagnosed with Addison's disease in April 2013. He was given Percorten and Prednisone. He recovered from this initial crisis and we gave him another shot of Percorten May 22.

At 3am on June 11, he would not eat his breakfast. This is VERY unlike him - he loves his food. He started vomiting and had extreme diarrhea. We thought that he was having another crisis related to Addison's disease. We took him into our vet and they checked his electrolytes, which all came back normal except for the K which was 3.5 (normal but low). They kept him overnight until the evening of the 12th and we brought him home.

The morning of the 13th, things were normal. He drank but refused to eat. We gave him 10mg Prednisone in AM and 10mg in PM in addition to Metronidazole in the AM/PM. I tried everything to get him to eat, he turned his nose up at everything. He also insisted on being alone, and was very restless. We noticed an excessive amount of drool later that night as well.

At around 11pm, we ended up at the Emergency vets. They took his vitals, everything appeared to normal but when they ran his electrolytes, the K was so low, it was at the bottom of the chart. They administered anti vomiting medicine and K via an IV until we could get him to our vet this morning.

Today, at the ER vet, he looked much better than he did but they said he was not responding to any treatment - his K was still not up and he was still vomiting.

We took him to our vet and they immediately got to work on him - he has a high white blood cell count, they did x-rays/ultrasound - no tumors, nothing abnormal or suspicious. They did tests for pancreatitis, that came back negative. They wanted to do a scope and see the whole GI tract but he is too weak and sick to put through something like that. Also, when he vomits, it's whole chunks of food (chicken) like nothing had been digested at all.

Our vet has said that his K is now at 2.8, and if we can get the diarrhea under control, we will be in a better place. He is on Sulcrafate now. All organs appear to be working and there is no damage so far.

I just don't know, we are very much at a loss. Has anyone had similar experiences, ideas? We love our dog very much and it's so hard to see him feeling so sick.

Who is your vet?

We're having issues with our dog again; throwing up everything. I'm sick of paying for bloodtests that show nothing abnormal except slightly elevated enzymes. They just keep rerunninig the tests. She's thrown up just about everything in the past 2 days.

Toxins? check your shoes, what are you bringing into the house? Any toxins in his world? Food changes? even the slightest toxins that WE can deal with can really impact a 70 pound pup!
Other than toxins, I can help no further.

We take our shoes off in the garage before coming into the house, he is not a nosy dog - never had issues with him getting into the trash or toilet bowl, etc. I will have vet check or run this by her just as a precaution. She has also checked for parasites, heartworms, etc.
I'm not sure about the blockage, she wants to do a scope but that's out of the question in his current state.

He's also losing his hair, shedding extremely bad. You can get a handful of hair just by patting him.

Toxins? check your shoes, what are you bringing into the house? Any toxins in his world? Food changes? even the slightest toxins that WE can deal with can really impact a 70 pound pup!

I thought this as well. Do you live somewhere that other people can access your dog? Neighborhood for instance?

So sorry to hear your dog is feeling bad. The fur loss could be from the Addison's, but the vomiting could be from as what Dowhat mentioned..a blockage, or twisting of the intestine. Or it can be from parasites. My dog got Coccidiosis last year, and was quite sick from that. Well wishes to your Pup.

Just saw this this AM on the forums page....my good friend JUST went through something very similar with her shepherd. She started at vet...went to MASH (for a while) and they ended up with a great vet at Dogs and Cats in Bowie that really understood Addison's the best (they call it the "masquerade disease" as it is so hard to get a handle on it and how to help). They saved their shepherd and he was SO sick but quite healthy now (although he does take prednisone to maintain and it gets adjusted). I sent her a note when I saw yours and she sent back:

Rugby has a typical Addison's
Her dog has regular Addison's
Where he gets a monthly injection of steroids where rugby is worse off and has to take it daily

Dogs n cats would be her best bet if she hasn't already been there

At least she's trying but Addison's is know as the masquerade disease
Hard to detect without spending a fortune and hard to manage with all if its issues until it is under control.

Plasma transfusions through Dog/Cat of Bowie were what saved Rugby's life. He is probably 5-7 years old as well.

Thanks for the suggestions everyone. The old vet we were using diagnosed Addison's right away with the ACTH test, however they were not properly managing or treating it. He needs monthly shots with prednisone daily, and they only prescribed the prednisone for emergency uses, or when he 'felt bad'.

We love our new vet, they have gone above and beyond. We went to visit him today and his potassium is hovering around 2.8 (a little low) but it is not dropping(this is very very good). He was given a tiny meatball this morning and was able to keep that down. When we saw him, he immediately perked up, tail wagging and made happy whine noises. We took him on a walk, and he even did a happy trot for a few strides. He got Belly rubs, kisses, the works. You can tell that he is MUCH better and he still has an IV but he no longer looks like he will keel over and die. We gave him another meatball and some water before we left and he was thrilled (he had no appetite before). I do not know about the diarrhea, the someone will visit later and hopefully have an update for us.

He is not out of the woods yet and we still have no idea what's wrong, except we do believe that this is not part of an Addison's crisis. We were seriously considering having to put him down because of how badly he was suffering and now I have some hope.

SNS, does it seem to be a little neurological with him? Last year at this same time, our girl came down with something that almost seemed like poison and affected her walking and digestive system. Vomitting and not wanting to eat anything. We thought it was the end. All her electrolytes came back normal so it was assumed not to be anything from the Addison’s.

Yet, our vet had one last ditch effort on putting her on antibiotics and upping the Prednisone to an outrageous dose per day for a couple weeks. It all worked in a few weeks and we still have her happy go lucky self. I don't know who you are using for a vet. I really would suggest Dr. O’Connor though.

I really can’t remember ALL the particulars of everything we went through that first year. We did have many, many, issues until we could get the Pred dose under control and she gets her DOCP shot every 28 days.
I really hope you get your guy back in shape and feeling better!!

He is getting stronger each day, so if he is doing well tomorrow - they will be able to do other tests to see if they can get to the root of the issue.

I have seen the Addy dogs website - even joined the facebook and yahoo groups they offer, trying to learn as much as I can. Very horrible disease but it can be managed, and we love our dog. He is just the cutest and smartest basset hound I've ever met.
