I have probably seen hundreds of "clips" of him, heard his show numerous times, seen how he badgers people into doing things that make them uncomfortable to get a laugh, exploits people with disabilities for a cheap laugh, seen how many of his longtime co-stars/friends have repeated trouble with the law and in their personal lives, saw his "autobiographical" movie, have seen his interviews over the years.
It could be an act, but if you keep up an act every single moment of your public life rather than say specifically during your shows/appearances then the act is you.

Like Clem said, many of the stunts he does are just mean. People like me can't even find it amusing or get some schadenfreude out of it because many times it's simple immature cruelty toward people who don't deserve it.

Regardless, whether he is all an act or not it's pretty shitty what he is doing with regard to meatloaf. It is the real person that decides whether or not to do that.
