I think it is fixed, not from the point of who wins and loses but to keep games exciting to generate more ad revenue, to keep people watching longer and not tune out in the 4th quarter. Far too many games are decided on last second field goals.

That discounts all the rule changes over the years and not just on the field. Free agency, how schedules are set. The NFL, if nothing else, has done a brilliant job of doing all they can to level the playing field. If it was fixed the Pats would not be so irritating to the league. They keep overcoming all efforts to keep them from winning so much.
