It is getting drawl, about 70 percent worth... no blockage at all, you can get on the roof and look down, its clear... the smoke goes both up the chimney and out the front of the fireplace at the same time.
Then the opening/taper/chimney size is incorrect and nothing short of tearing it all down and putting up a new one will correct it. Too bad. Too many people putting up fireplaces that don't know what they are doing. They wind up being decorations because they are not functional. There are several things up don't go cheap on, sushi, guns, surgery, and some others. Fireplaces are included in others.

If you aren't getting a decent draw from the chimney and you can get on the roof and look down it then the chimney is probably too short.
How far above the peak (highest point) of the roof does it extend?

 that woud require a source of air coming into the middle of the chimny.

thanks, I've checked, as she did and a professional did, but no air was coming out of the chimney other than top of the chimney and smoke billowing out into and through the house.

thanks 2A its an old house. but its clear from top to bottom, the previous ownes had an insert with no problems, but took it with them.
:offtopic: but..... If she bought recently and the house was shown with the insert installed and not SPECIFICALLY mentioned on the sales contract that they were taking it with them she has recourse there.... They have to leave it or replace it with one.

thanks all for the advice, but I just read greenteas thread about the accident, and I can't help but feel selfish for something soo petty concerning an insert compared to her situation, so I'd appreciate it if folks could hold off on their posts on here and bump her thread for when or if she comes back. She needs help and input soo much more than I do right now..
I want Greentea to know this is a supportive forum and for her to easily find her thread in this particular forum without mine clogging the airwaves... does that make sense? I feel bad about all of this. I probably should have read her thread first before posting this in the same forum. Thanks all. There's no immediate rush, but if anyone has any inserts available that meet the criterion please pm me. Thanks!

Dems, don't be so quick to discredit your thread. You may think it's all about simply trying to find a low cost insert for a fireplace, but these people are trying to offer you life saving information. Read the link posted by Ken King.

Quit beating around the bush, Dems, can't you tell that Aps(with all his replies of concern) wants you to invite him over to take a look at the fireplace. Don't be so coy, go for it.

You really should pay attention to what everyone is trying to tell you and have a real professional look at it. It sounds like a basic flaw in the design of the chimney and/or the firebox. It makes no difference if you can see from top to bottom, that just assures you that there are no animals living in the chimney.

the house is old, they have been using it for hundreds of years, there is NO design flaw or the house would never have been inhabited. No heat = no inhabitants

Since it is an old house, it has probably been remodeled which may have included weather stripping and more insulation which would make the house more air tight. That can cause a back draft. Try slightly opening a window to see if the smoke still comes back into the house. If it doesn't, then your friend needs to come up with a way to proved air for combustion without cooling off the house.
